Managing a chronic musculoskeletal condition—such as persistent low back pain, osteoarthritis, or a recurring injury—can be both physically and financially challenging. Many Australians are unaware that Medicare may provide a rebate for osteopathic treatment under a specific scheme.
This blog explains the Chronic Disease Management (CDM) plan and how eligible patients can access subsidised osteopathic care with a referral from their General Practitioner (GP).
What is a Chronic Disease Management (CDM) Plan?
A Chronic Disease Management (CDM) plan is a coordinated care plan developed by your GP for patients who have a medical condition that has been (or is likely to be) present for six months or longer.
These plans are designed for conditions such as:
Musculoskeletal conditions (e.g., chronic low back pain, osteoarthritis)
Diabetes
Cardiovascular disease
Asthma
Arthritis
And other chronic health issues
The goal of a CDM plan is to provide a structured approach to your care, which can include referrals to allied health professionals like osteopaths.
How Can a CDM Plan Help with Osteopathic Treatment?
If your chronic condition has a musculoskeletal component that would benefit from osteopathic care, your GP can include this in your plan. This referral allows you to claim a rebate from Medicare for a set number of osteopathy sessions each calendar year.

The Referral Source: You must have a valid referral from your GP. You cannot self-refer under this scheme.
The GP's Role: Your GP will assess if you are eligible for a CDM plan and, if appropriate, create one for you. They will determine if osteopathic care is a suitable part of your management.
Number of Sessions: A CDM plan typically allows for up to five subsidised allied health visits per calendar year. It's important to note that these five visits are shared across all eligible allied health providers (e.g., if you also see a physiotherapist or dietitian, those visits count toward the total five).
The Rebate Amount: Medicare provides a rebate for each session, not full coverage. The rebate is a set amount, which is often less than the clinic's consultation fee. As of 1 July 2025, the Benefit: $61.80. You will need to pay the full fee at the time of your appointment, and we can use your Medicare card and valid debit card to process the rebate on the spot. Please note that a credit card will NOT work to claim the rebate.
A Step-by-Step Guide to Accessing Care
Book an Appointment with Your GP: Schedule a consultation with your regular GP to discuss your chronic condition and your interest in osteopathic treatment as part of your management.
Discuss a CDM Plan: Ask your GP if you are eligible for a Chronic Disease Management (CDM) plan. Explain how your condition impacts your daily life and mobility.
Request an Osteopathic Referral: If a CDM plan is created, ask your GP to include a referral for osteopathic services. Ensure the referral is made out to our clinic or to an "Osteopath."
Bring Your Referral to Us: Once you have your referral from your GP, contact our clinic to book your appointment. Please bring your referral and your Medicare card to your first session.
