Once your booking is confirmed, you’ll receive a welcome email with everything you need to get started, including an Informed Consent Form and Questionnaire about your issue. You can complete these online or arrive 10 minutes early to do it on paper.

• Wear appropriate clothing so we can assess and treat the relevant area. Loose fitting clothing is best. Disrobing is generally required so that we can assess and treat effectively, however, we can always find alternatives.

• Any relevant medical documents or prescriptions, including imaging reports.
