What Actually Happens During Your First Osteopathy Consult?

Booking your first osteopathy appointment can feel like a step into the unknown, especially if you've never seen an osteopath before. Will it hurt? Do you need to undress? How long does it take? This guide walks you through what a typical first consultation involves at Nudge Osteopathy, so you can arrive feeling informed and comfortable.

As always, this information is general in nature. Exactly what happens in your appointment will depend on your individual presentation and your osteopath's clinical judgement, so it's best to confirm specific details with your clinic when booking.

Before You Arrive

It's worth turning up a few minutes early to your first appointment. If you haven't completed the online forms in the Confirmation Email, we can provide paper copies to fill out. It can help to bring along:

  • Any relevant scans, imaging reports, or specialist letters

  • A list of current medications

  • Details of past surgeries or significant injuries

  • Notes on your goals for treatment (for example, returning to a particular sport or activity)

In terms of clothing, loose, comfortable clothing that allows easy movement is generally recommended, as your osteopath may ask you to move through certain positions during the assessment. Depending on the area being examined, you may also be asked to remove some clothing or change into a gown so we can properly assess the affected area — you're always welcome to discuss your comfort level with your practitioner beforehand.

How Long Does a First Consultation Take?

Initial consultations are generally longer than follow-up visits, to allow time for a thorough history and assessment. An initial consultation commonly runs for 60 minutes, with follow-up appointments typically shorter. If you have any questions, please don't hesitate to ask.

You don't need a referral from a GP to book — osteopaths are recognised as primary contact practitioners in Australia.

Step 1: Case History

We will start by asking questions about your general health and the reason for your visit. This typically includes:

  • Your current symptoms and how they're affecting your daily activities

  • Relevant medical history, including past illnesses, injuries, or surgeries

  • Any medications you're currently taking

  • Lifestyle factors such as work, exercise, and sleep, which may not seem directly related to your presenting complaint but can help us understand the broader picture.

Being open and thorough here helps us build an accurate picture of your health and identify whether osteopathy is likely to be appropriate for your situation, or whether you should be referred elsewhere.

Step 2: Physical Assessment

Following the history, we will generally carry out a physical examination relevant to your complaint. This may include:

  • Observing your posture and how you move

  • Passive and active movement testing (for example, your osteopath may ask you to bend, reach, or lift a limb)

  • Orthopaedic or neurological tests where clinically relevant

  • Palpation — feeling the affected joints, muscles, and soft tissues to assess their condition

Because osteopathy takes a whole-body approach, we may also examine areas beyond where you feel pain. For example, someone presenting with knee pain might also have their ankle, hip, and lower back assessed, as these areas are likely functionally related.

Step 3: Treatment (If Appropriate)

If treatment is appropriate on the day, we will explain the proposed approach and technique options before proceeding, and will seek your consent. Techniques may include:

  • Soft tissue massage and stretching

  • Joint articulation and mobilisation

  • Muscle energy techniques

  • Other manual therapy techniques appropriate to your presentation and your practitioner's training, which may include dry needling and high velocity, low amplitude thrusts.

Some people feel mild soreness for a day or two after their first treatment, which is a normal response for some individuals, however, treatment should never feel more than mildly uncomfortable, and you should always tell us if something doesn't feel right so we can adjust our approach.

Not every first appointment includes hands-on treatment. In some cases, we may recommend further investigations (such as imaging) or referral to another healthcare provider before proceeding.

Step 4: Advice and a Plan Going Forward

Towards the end of the appointment, we will typically discuss our findings with you and outline a suggested plan, which may include:

  • Home exercises or stretches

  • Posture or ergonomic advice

  • Lifestyle suggestions relevant to your goals

  • A recommendation on follow-up appointments, if appropriate for your situation

We will always explain our reasoning and give you the opportunity to ask questions, rather than presenting a single fixed plan without discussion.

A Note on Consent and Comfort

Registered osteopaths are required to practise in line with AHPRA's shared Code of Conduct, which includes obtaining informed consent before examination or treatment and respecting a patient's right to decline any part of the assessment or treatment at any time. If you're ever unsure about a technique being proposed, it's entirely appropriate to ask us to explain it, or to decline.
